Proponent must submit the following eligibility documents
Proponent must submit a notarized Omnibus Sworn Statement for Philippine Government Procurement
Proponent must deliver the items in PSHS-CLC
Proponent must be familiar with CDC policies in bring in and bring out policies
Proponent must be familiar with Government Transaction
