Number, and PCAB license during submission of quotation
Proponent must conduct the repair onsite at PSHS-CLC
Proponent must submit Income/ Business Tax Return, Omnibus Sworn Statement and Performance Security after receiving the Notice of Award prior to Contract Signing
Contract price will be subject for retention
Proponent must complete the repair service within (60) sixty calendar days
Proponent must be familiar with CDC regulations and policies including gate pass (entry/exit)
Proponent must be familiar with Government Transactions
