Note: Some tables have gas tanks installed, electrical circuits and/or waterlines. Tables also have equipment that needs to be handled with care and one table will need 3-6 persons to move/lift (see attached pictures for your reference). Proponent can coordinate with the general services unit for these notes.

Scope of work:
-Cutting down of tables' steel legs, and additional wooden base support, leaving at least an inch from the existing measurements
-Retouch paint following the existing paint color of tables’ legs
-Rubber stopper/Levelling screws at the end of the tables' legs should be replaced or retained
-Cutting down of chairs' steel legs, at least 6-10 inches cut
-Retouch paint following the existing paint color of chairs’ legs
-Rubber stopper/footing at the end of the chairs' legs should be replaced

Proponent must conduct a site visit at PSHS-CLC
Proponent must have main office/satellite office in Region III
Proponent must complete the repair service within (60) sixty calendar days
Proponent must be familiar with CDC regulations and policies
Proponent must be familiar with Government Transactions
