Proponent must submit Mayor’s/Business Permit, DTI/Sec registration, BIR Registration,
PhilGeps Registration Number/Certificate and Sample Official Receipt
Proponent must be familiar with CDC regulations and policies
Proponent must submit Omnibus Sworn Statement on or before signing of Contract of Service
Proponent must be familiar with Government Transactions
